Waiver of Liability *

In consideration for my volunteerism in programs of River City Community Development Center (R CITY), on behalf of myself, my family, my heirs, and my assigns, I hereby release R CITY's volunteers and employees from liability for injury, loss, or death to the above mentioned participant while using any facility or equipment or in any way associated with participating in the activity of being a participant in R CITY programs. This release from liability also applies to River City Community Church, including but not limited to its facility at 3709 W Grand Ave, and its volunteers and employees.

Release of Information *

I hereby consent that audio, photographs, video/film and narratives of me may be posted on the websites of River City Community Development Center and/or River City Community Church, as well as the social media pages associated with either of these entities. I reserve the right to contact the webmaster (info@rcitycdc.org) if I desire to have any of these publications removed from the websites after publishing. Furthermore, I hereby consent that such photographs, audio, and video/film are the property of R CITY (NOT FOR SALE/PURCHASE) who shall have the right to duplicate, reproduce, and make other uses of this media as they may desire free and clear of any claim or compensation whatsoever on my part.
